Home Safety, Equipment, and Modification Assessment – For those experiencing challenges with living independently due to injury, disability, or age, our experienced occupational therapists will come to your home to complete a safety assessment. Your functioning in the home will be assessed, as well as the physical environment of the home. Recommendations are made for modifications to the home, and adaptive equipment to enhance your ability to live independently in your home.
Assessment for home support services and personal care attendant needs - An in-depth evaluation of an individual’s ability to perform their activities of daily living (ADLs) at home. The assessment report will identify all of the assistive needs of the individual and the need for providing Personal Care Attendants and Home Support Services.
